Drugs to be Avoided or Used with Caution in Breast-feeding

1. Gastro-Intestinal system  
     Atropine May possibly have anticholinergic effects in infants
    Laxatives  
      Anthraquinones  Avoid; large doses may cause increased gastric motility and diarrhoea, particularly cascara and danthron
     Phenolphthalein Avoid; increased gastric motility, diarrhoea, and possibly rashes
2.Cardiovascular system  
     Amiodarone Avoid. Present in milk significant amounts.
     Beta - adrenoceptor bloking
     drugs
Monitor infant; possible toxicity due to beta blockade but amount of most beta-blockers excreted in milk is too small to affect infant.
    Oral anticoagulants Risk of haemorrhage; increased by vitamin-K deficiency. Warfarin appears safe but some authorities consider breast-feeding is contraindicated during therapy. Phenindione should be avoided.
3. Respiratory system  
    Aminophylline Irritability in infant has been reported.
    Clemastine Drowsiness in infant has been reported.
    Cough mixtures containing
    iodides
Use alternative cough mixtures.
     Ephedrine, Pseudoephedrine Irritability and disturbed sleep reported with ephedrine. Significant amounts of pseudoephedrine in milk.
4.Central nervous system  
     (a) Hypnotics and sedatives
