Actions
Flemoxin
is a bactericidal antibiotic which inhibits bacterial cell
wall synthesis and causes lysis of susceptible bacteria. It
predominantly inhibits glycosidase responsible for polysaccharide
chain (side wall) formation and this explains as to why amoxycillin
is greater and more rapid bactericidal than ampicillin. On
oral administration absorption is rapid and greater (90%)
compared to ampicillin and is less variable being more stable
to gastric acid; it diffuses readily into most body tissues
and fluids with concentrations in sputum, biliary fluid, middle
ear fluid and nasal sinus greater than ampicillin. Reported
to be more active against Streptococcus and Salmonella species
though less active against Shigella species.
Advantages
-
Absorption after oral administration is rapid, almost complete
and less variable
-
Highly
stable in presence of gastric acid and can be administered
without regard for meals
-
Peak
plasma concentration twice as high as with the same dose
of ampicillin
-
Greater
and rapid bactericidal actions
-
Excellent penetration with higher concentration in
most body tissues and fluids
Lesser
incidences of side effects.
Adverse Reactions
Flemoxin is
generally well-tolerated and incidence of side effects are
less and usually mild in nature. Common adverse effects like
nausea, vomiting, diarrhoea, skin rash, urticaria, leucopenia,
neutropenia, agranulocytosis are reported to occur and are
usually reversible on discontinuation of the drug.

Indications
Flemoxin is useful in the treatment of ENT and respiratory tract infections
caused by Streptococci, Pneumococci, non-penicillinase producing
Staphylococci, H. influenzae; genitourinary tract infections
caused by E. coli, Proteus mirabilis, Streptococcus foecalis;
gastrointestinal tract infections caused by Salmonella species;
skin and soft tissue infections caused by Streptococci,
non-penicillinase producing Staphylococci, E. coli; venereal
infections like gonorrhoea and primary syphilis. As prophylactic
in infective endocarditis and in the treatment of
dento-alveolar abscess
Contraindications
Flemoxin
should not be administered to patients with histories of
hypersensitivity to penicillins
Precautions
Flemoxin
should be used with caution in patients with known histories
of allergy. The drug should be discontinued if allergic reactions
occur. Patients with impaired renal functions must be given
reduced dosage. Overgrowth of non-susceptible organisms, including
fungi, may occur. Periodic assessment of renal, hepatic and
haematopoietic functions should be made during prolonged therapy.
Safety in human pregnancy has not yet been established.
Drug interactions
Contraceptive failure and pregnancies are reported to occur
in a number of cases due to concurrent use of penicillins
(amoxycillin) and oral contraceptives.
